About this card's ① st effect:
This effect is a Trigger Effect, which can be activated in the Hand.
You cannot activate it during the Damage Step.
When this effect resolves, Special Summon this card from your hand. Then, strictly afterwards, return the targeted monster to its owner's hand. (These are not treated as having happened simultaneously.)

About this card's ② nd effect:
This effect is a Trigger Effect, which can be activated in the Graveyard.
This effect can only be activated if both players have 5 or more cards in their Deck.
You can also activate it during the Damage Step.
When this effect resolves, send 5 cards from the top of each player's Deck to the Graveyard. Afterwards, if Exchange of the Spirit is in your Graveyard, you can then choose to Set 1 Trap Card from your Graveyard to your field. (These are not treated as having happened simultaneously.)
If either player has 4 or fewer cards in their Deck at resolution, they send their entire Deck to the Graveyard.
